ceph-debug-docker: add --flavor option

* add --flavor option, which is "default" by default, so one can, for
  example, pass "--flavor crimson" to ceph-debug-docker
* extract $repo_url to avoid repeating the shared bits between centos
  and debian derivatives envs.
