doc/cephadm: improve "Upgrading Ceph" (main)

This PR makes a couple of minor improvements to the text under the
top-level section "Upgrading Ceph" in the "Upgrading Ceph" chapter of
the cephadm documentation.

This one, mercifully, contains only a couple of changes.
