doc: split up SubmittingPatches.rst

Split the existing SubmittingPatches.rst into three files:

* SubmittingPatches.rst (general guidelines, for GitHub/master branch)
* SubmittingPatches-kernel.rst (for kernel patches)
* SubmittingPatches-backports.rst (for backports)
