mon/PGmap: Add warn description for bluestore fragmentation

Add a user friendly description for
BLUESTORE_FREE_FRAGMENTATION warning.  This is displayed
in `ceph -s` and `ceph health detail` to help the user
understand what BLUESTORE_FREE_FRAGMENTATION means.
